Please arrive 10 minutes prior to your appointment. If you are a new patient, please arrive 15 minutes early to fill out the necessary paperwork.
Your copay is calculated according to your dental insurance benefits, and we will contact you prior to your appointment to discuss. You will also receive your treatment plan by email and regular mail. Payment is due at time of service.
- If you are a new patient, please bring referral cards or X-rays from the dentist who referred you to our office.
- Bring your ID, medical, and dental insurance cards.
- Do not drink or eat anything (not even water) EIGHT HOURS prior to your appointment. This is for your safety.
- You must be accompanied by a person over the age of 18 who will remain at our office during your entire surgical procedure. They must be able to drive you home.
Please remove any nail polish off your index fingers prior to your surgical procedure.
Please contact the office at least 24 hours in advance to reschedule to avoid a cancellation fee.
A parent or legal guardian must accompany all minors.
We generally recommend seeing Dr. Kur for a consultation prior to treatment. However, for wisdom teeth, we can schedule a consultation and surgery on the same day.
